Well first we would need to know what software you use:
    1. Traktor = you cannot. Traktor does not have a flexible beatgrid and hence cannot do BPM change gridding.
    2. Serato has some good demos on their website to their flexible beatgrid
    3. Cross does it automatic in 90% of the cases

The question is also: Why would you need such a transition track?
Be flexible forget the beatgrid and the sync button and do some fancy tricks on your own.
I regularly change BPM between 90 to 140… no need for transition tracks.

I’d probably suggest you perfect manual beatmatching in this case.

I use sync, but this is one of those occasions where it won’t work well.

If you’re using Traktor (which can’t do more than one BPM in a track) what I do is beat grid the track to the second (after transition) BPM, so I`ll set the beat marker on the first downbeat at the new transitioned BPM.  I do this because it’s probably easier to manually beat match the intro to your current BPM (probably won’t be more than a few BPM off), then you can sync back in after the transition and go from there.

ok thanks for all the tips… might just do the normal trickery from one bpm to the other 🙂

How I do it is that I first label the tracks title with the BPM’s it transitions to and from.  for example “130-72 Holy Grail” by Jay-Z.  This makes it easy to find in my library and obvious where it transitions into and from.  Then when I beatgrid the track I only beat grid the initial intro of the song, so in this example the part where it’s 130bpm.  This makes it easy to beatmatch the transition song with the outgoing song.  Then when I need to introduce a new song over the transition song, ill just set the pitch fader to the original bpm position and will know that it will be the second bpm listed in the title, so again in this case it will be 72bpm.  I just set new track to 72 bpm and there you go, should be beatmatched already.
