    Hi Mehul, welcome to the forums.

    Few comments:

    1) Think in what forum you post your question. You posted this in forum for questions about the actual form (use/workings). So I moved it to this forum The DJ Booth, which is for all DJ related questions that don’t involve hardware or software.

    2) One of our posting guidelines is to behave (highly) respectful. And as we all know SHOUTING is not very respectful. Your header was ALL CAPS, which is considered shouting. So I corrected that.

    Please try to follow the rules, we (moderators) don’t need the extra work 😀

    The laws are going to vary from state to state AND whether or not you are an employee versus working as an independent contractor. In many states, it is illegal for minors to be employed by “entertainment” venues under the age of 16 (movie theaters, etc.)… mainly because the category is so overused for other establishments. If you are working as an independent (meaning, you have your own business and are being paid per job) then the only laws you need to worry about are if you are legally allowed to be in the establishment. What you may want to consider is having your parents form an LLC, negotiate the gig through the LLC, and then you act as an employee of the LLC. This will protect you, and your family, from any lawsuits that may arise and will eliminate you from the employee laws.
